I Couldn´t Do The Request For Live Special OPS! It Appeared This Message: "Whoops Looks Like Something Went Wrong"

Hi, dear:

This message: "Whoops looks like something went wrong", appeared when i wanted to do request for live special OPS!
So i did i offline and i wroting the why reason. But i need to do the live special OPS, because it is a requirement for International Fellowship in Thailand which you request 

Please if somebody can help me, thank you so much!
