Feedback Form, Please Help

Hello eveyone,
I have been trying to submit my feedback form, I filled everything in but when it comes to save the changes at the end it appears the following message:

Cannot submit the feedback. Please try again later. Reason = SQLSTATE[23000]: Integrity constraint violation: 1062 Duplicate entry '0-0--internet_location' for key 'member_id' (SQL: insert into `member_questionaires` (`qid`, `member_question`, `updated_at`, `created_at`) values (9477, internet_location, 2014-03-20 10:45:46, 2014-03-20 10:45:46))

I have tried later but it keeps doing the same thing. Could you help me please?
